this literally does not mean "literally"

Today's Spanish word of the day from Transparent Language is actually a phrase: "al pie de la letra", which literally means "to the foot of the letter". Both Google Translate and SpanishDict translate it "to the letter". My question is: whose foot is it, and what is it doing there?
