I called three reputable vets including my own and no one has heard of it. Possibly because I am Tennessee and far from any border?

Try these.
International Health Certificate. A completed and signed International Health Certificate must be endorsed by a USDA (U.S. Dept. of Agriculture) APHIS (Animal & Plant Health Inspection Service) Veterinary Services area office in order to be valid. To obtain the USDA endorsement of an International Health Certificate or any other documents relating to traveling with your pet, the documents must be completed by an APHIS accredited veterinarian who certifies animal health status, conducts tests, and records test results for the individual animals being exported

The last reference to Form 77-043 I can find is from 2004.

I think it is old info. The current Health Certificate you get from the Vet verifies the dog's rabies shot records and general health. They are valid for domestic and international air travel.

Are you flying the dog from Tennessee to Mexico?

CERTIFICATE

The certificate is called:

"Certificate for International Movement of Small Animals"

Vets can access the form on the web:
www.cdfa.ca.gov/ahfss/Animal.../pdfs/small_animal_certificat...

They used to come on pink paper, but now vets print them off the internet.

When I try to access this link I get an error message. I'm just planning on bringing my dog's recent vaccination paperwork. Is that enough? Thanks.

Bring all his/her papers. I have mine seen within 10 days of going in by a vet and they have the paperwork (or should). I would be VERY upset if we could not return with our pet. We have never b een asked for the papers but you know...the first time you don't have them you will be questoned. Have fun. Our dog loves going.

:?::?:Why do we need this form? Only once have I been asked for any type of paperwork for my dog when entering the Baja. Just showed his rabies shot paper and a copy of his County dog license.
