FMT at otay?
my buddy and his son will be flying to la paz next month from tj. he needs to get them both tourist visas and needs directions to the office at otay.
he's getting dropped off and walking across, then grabbing a cab to the airport.

If you are talking about flying out of the TJ Airport,
as of last Summer, you get and pay for the fmt/fmm
at the Airport, not at the Otay crossing. I dropped
some people off who flew TJ/Cabo on Volaris last June.
Mex IMM at the TJ Airport issued the fmt, they paid the
fee at the Aeromexico counter inside the Airport,
Mex IMM the stamped the fmt and allowed them into
the boarding area. Unless things have changed,
and I don't believe they have, you get the fmt at
the Airport. The form they issue for flying differs
slightly form the port of entry form, that is
the airport form states it is for flying, I don't believe
the otay crossing can or will issue an fmt for flying
I believe the Otay crossing mex IMM will tell
them to get the FMM at the airport

What sancho says is correct, with the exception that you would be able to use the form obtained at the border, if you so chose, to also board the
flight. That said, it just seems more convenient to do all the paperwork at the airport.

will they have to pay for it? is it surrendered on the return flight or do they keep it for 180 days?

Since this is Mexico, anything goes. Ordinarily, when one is boarding a return flight to the States, they confiscate the document. However, since they
are going to be boarding a domestic flight, I'm not so certain that they will have to surrender it. Inasmuch as if they have their visa confiscated
upon boarding, they will, in effect, be violating the law upon arrival in TJ, as they will not have a visa, but will have over-stayed the 72 hour
limit. But, once again, consider where this is happening.
