Good luck! They are all notorious crooks at the airport. The insurance they push is cost prohibitive, way more than the rental fee. Check the vehicle for damages and document it on the contract. Check for a spare with air, jack, lug wrench. If cost is a concern, best bet would be to reserve and pay online at an agency away from the airport. Local buses regularly service SJD into town.
No longer can you deny their insurance, (local rules), they get you good there.
I've used Eurocar all over the Republic and left a credit card deposit in lieu of their insurance. Super reasonable weekly rentals online, less than $100. When paid online the agency has no idea what you are paying. Not for everybody, but works great for me.

My experience is different at SJD. With Budget, Dollar & Hertz you are covered for Liability but they will try to sell you more. Read the fine print when you book, it is quite clear. Take a copy and show them the part that says it is provided. Other coverage can be declined if you want to take the risk. Most CC's cover collision.
The only time this did not work was when I didn't have a copy of the terms and conditions with me.

Me too! Did you misread the quote? Sounds like it is a pipe dream.

From back in the 1980s, we have relied on Avis out of SJD. They are not located on the airport property, but provide quick van service to their office just South of the airport.

Never, ever had a problem with Avis there, although I must say that they have a resident time share dude who can be a bit annoying.
